Instructions:
1. Read about the diseases on this handout.
2. Look at the immunization schedule given to you by your instructor.
3. Copy the name of each disease and paste it beside the age that requires this immunization.
4. Add a comma and a space after each disease you paste. For example,
2 months – DIPTHERIA, POLIO, HIB, PERTUSSIS, TETANUS
